Participants may find some aspects of Korean Buddhist Martial Arts Association (KBMAA) programming physically and/or mentally demanding. Our programming can and often does include rigorous physical activity in the context of a vigorous schedule. I realize that I may freely decline to participate in any activities that in my sincere judgment I deem dangerous to my well being. I realize that I may consult with a teacher at any time to resolve any difficulties I might have.

I agree to release the Korean Buddhist Martial Arts Association and it's parent legal entity, the United Buddhist Church Inc., their agents and affiliates from all liability from any injury I may suffer as a result of my membership and participation in KBMAA activities and to indemnify the same for any injury to others caused by me.

I understand that making application to KBMAA via this form does not constitute automatic acceptance into the membership of the KBMAA, and that in addition to the above information, the KBMAA may request further information regarding the applicant that will be required for final membership determination, to include the possibility of a current demonstration of skill via video or in person performance.

All decisions regarding the admittance of applications are issued personally by the current President and Grandmaster, and are thus to be regarded as final.
